Welcome to the Commonwealth of Kentucky’s Federal Post Card Application ("FPCA") online form wizard. This wizard will walk
you through completing the FPCA, which can be used to apply for an absentee ballot and register to vote in the Commonwealth of Kentucky.
- While completing the form, you will be prompted if any required fields, designated by an asterisk (*), have not been populated.
- When you have completed the wizard, you will have two options:
If you choose to receive your ballot via email/online, you will receive an email from an online eBallot Delivery system. Please be sure to add the KYSupport@secureballotusa.com domain to your trusted email senders.
- Sign your completed form digitally using your mouse and submit it electronically. (You can also print a completed copy,
including the signature, for your records.)
- Print out your completed form and sign it manually, then send it to your county clerk via mail, email or fax. You will be provided with your specific county clerk info, including mailing address, at the end of the wizard.
Your application may be subject to public inspection or copying pursuant to the Kentucky Open Records Act, KRS 61.870 to 61.884, and KRS 116.095. However, your electronic mail address and social security number are exempt from disclosure.
